چکیده

In 1950s the Government of India under National Malaria Eradication Programme (NMEP) now National Vector Borne Disease Control Programme, launched massive insecticidal spraying with DDT for malaria vector control and kala-azar came to a halt in Assam as a collateral benefit of malaria eradication programme. As a result, there were no further reports of kala-azar from the northeastern region except some sporadic cases. We report here the re-emergence of kala-azar in Assam in outbreak form. An outbreak of fever in Chapaidang village, Chandrapur area of Kamrup Metro District in Assam was investigated during February 21 to 26, 2008. Chapaidang village with a population of about 800 is located on a foothill and is 20 km from Guwahati city. The local medical practitioner from the area recorded the presence of many febrile cases of longer duration presented with splenomegaly, anaemia, weakness and loss of weight not responding to antimalarials therapy and requested epidemiological investigation. Accordingly, we investigated the cause of febrile illness. During houseto-house visit all suspected subjects were enrolled after taking informed consent. Information like duration of fever, visit to other areas, other known ailments, their professions and recent treatment taken were recorded on a pre-designed proforma. Demographic data and previous record of Primary Health Centre revealed that the area was endemic for malaria. Looking into the patients’ clinical details, topography of the place and the key informants (doctors) description of cases, a primary suspicion of kala-azar was made. Ten randomly selected houses having congenial environment for vector (Phlebotomus argentipes) propagation were chosen for sand fly collection from the area.

Comparison of an rK39 dipstick rapid test with direct agglutination test and splenic aspiration for the diagnosis of kala-azar in Sudan.

We compared an rK39 dipstick rapid test (Amrad ICT, Australia) with a direct agglutination test (DAT) and splenic aspirate for the diagnosis of kala-azar in 77 patients. The study was carried out under field conditions in an endemic area of north-east Sudan. The sensitivity of the rK39 test compared with splenic aspiration was 92% (46/50), the specificity 59% (16/27), and the positive predictiv...

Prospective evaluation and comparison of the direct agglutination test and an rK39-antigen-based dipstick test for the diagnosis of suspected kala-azar in Nepal.

The diagnosis of visceral leishmaniasis (kala-azar) remains difficult in rural endemic areas and practical and reliable tests are badly needed. Two serological tests, the Direct Agglutination Test (DAT) and an rK39-antigen-based dipstick test, were compared to parasitological diagnosis in a group of 184 patients presenting at a tertiary care centre in south-eastern Nepal with a history of fever...

Objectives: A rapid immunochromatographic dipstick test (ICT) has become available for the qualitative detection of anti-Leishmania antibody using recombinant rK39 antigen. This study was carried out at the department of Microbiology of Rajshahi Medical College, Bangladesh, in order to evaluate the diagnostic potential of immunochromatographic dipstick test. Methodology: A total of one hundred ...

Potential of direct agglutination test based on promastigote and amastigote antigens for serodiagnosis of post-kala-azar dermal leishmaniasis.

Post-kala-azar dermal leishmaniasis (PKDL) is a dermal complication, a sequel to kala-azar. Diagnosis of PKDL presents a challenge due to the low parasite burden in the lesions. The direct agglutination test (DAT) based on promastigote and amastigote antigens of Leishmania donovani of indigenous isolates was developed to diagnose PKDL, and the results were compared with those of the rk39 strip ...
